This prospective cohort study involves people with type 2 diabetes recruited from 15 urban community health centers. All participants received integrated care according to standardized protocols, delivered by collaborative team of tertiary hospital specialists and community primary care providers. It included visits for medication adjustments at least four times/year and individualized diabetes self-management education.

The investigators intend to present data on risk factor control and cardiovascular and mortality outcomes, and on the predictive power of neck circumference for cardiovascular endpoints.
